As Aftercare Coordinator, Katie Suss plays a vital role in supporting clinicians and ensuring a smooth transition for patients leaving Silver Hill Hospital’s Residential Programs. She is responsible for patient discharge and aftercare planning, which includes fostering and maintaining relationships with outside programs and providers and facilitating communication between internal and external teams.
