Silver Hill Hospital, Inc., a nonprofit hospital for mentally ill persons, and nonprofit Silver Hill Health, Inc. (the “Applicants”) are filing a Certificate of Need application pursuant to Section 19a-638 of the Connecticut General Statutes for Silver Hill Health, Inc. to become the parent of Silver Hill Hospital, Inc. by becoming the sole corporate member of Silver Hill Hospital, Inc. (the “Proposal”).  The Applicants have the same address, which is 208 Valley Road, New Canaan, CT 06840.  The total capital expenditure for the Proposal is zero dollars ($0).
