Jenny Grace Jablonka, LMHC, LPC
As a Senior Clinical Coordinator dedicated to The Steward House, Jenny Grace Jablonka is often the first point of contact for executives and professionals entering this program. Jenny works alongside program director, Petra Pilgrim, MD, and other departments to ensure seamless transitions for all individuals entering treatment. She facilitates pre-admission phone screenings as well as in-person assessments on the inpatient units. In addition to these responsibilities, Jenny serves as a team social worker, coordinating care for Steward House residents.
Jenny has been with Silver Hill Hospital for more than a decade, beginning as a Senior Admissions Clinician and then Assistant Director of Admissions for nine years, before transitioning to a social worker role in the Argent Program. Prior to her career at Silver Hill, Jenny worked as a therapist at a children’s psychiatric hospital and as a program referral specialist for an outpatient mental health clinic.
Jenny’s dedication to patients does not stop when they leave Silver Hill. She has cultivated strong relationships with referral sources to provide and coordinate safe and therapeutic discharge plans for patients leaving Steward House to support them through their entire care journey.
