Care Navigation and Support Program
Overview
The Care Navigation and Support Program guides patients and their families through the complex and sometimes overwhelming transitions following inpatient, residential and outpatient mental health programs. The Care Navigation and Support Program eases the burden on patients and their families during this stressful time.
About the Program
Care Navigation and Support Program provides support that is customized to the individual needs of patients and their families. It is designed for those who need additional case management or community support during and following inpatient, residential, sober living, or outpatient care. Patient can be referred directly from the community or as a part of discharge planning from other programs.
The Care Navigation and Support Program takes a holistic approach to an individual’s recovery. Examples of care navigation include linking mental health services, primary health care, housing, transportation, employment, social relationships, and coordinating community-based services. Silver Hill will also provide education to clients and families about resources available in the community.
What to Expect
Participants in the Silver Hill Hospital Care Navigation and Support Program will receive customized care navigation, coordination, and support as clients transition from inpatient, residential or outpatient mental health care. The Care Navigation and Support Program seeks to reduce hospitalizations and crises through an individualized approach to addressing clients’ needs for recovery, , functioning and family needs during vulnerable times.. Silver Hill will contact the client weekly, maintain contact with necessary providers or agencies, and provide ongoing assessment and communication of recommended services.
Here are some details about the program:
- Patients may enroll for a minimum of one month, but a three-month minimum is recommended to ensure stable support.
- Service may be provided virtually or in-person.
- Family involvement is strongly encouraged.
- Services are not direct mental health treatment but will help a patient along the continuum of care.
