Care Navigation and Support Program

The Care Navigation and Support Program guides patients and their families through the complex and sometimes overwhelming transitions following inpatient, residential and outpatient mental health programs. The Care Navigation and Support Program eases the burden on patients and their families during this stressful time.

About the Program

Care Navigation and Support Program provides support that is customized to the individual needs of patients and their families. It is designed for those who need additional case management or community support during and following inpatient, residential, sober living, or outpatient care.
